Photo Release Consent Form

I, hereby grant permission to Worldsculpt Inc to take and use photographs of my child,  for the following purpose(s):

• Educational

• Promotional

• Editorial

• Commercial

 

I understand that the photographs may be used in various media platforms, such as websites, social media, newsletters, brochures, posters, etc. I also understand that I will not receive any compensation or royalties for the use of the photographs.

I release  Worldsculpt Inc from any liability or claims arising from the use of the photographs. I acknowledge that I have read and understood this consent form and that I have the legal authority to sign it on behalf of my child.
